Physician-led ibogaine care, built around screening, cardiac safety, and the weeks that follow
Ibogaine treatment is a medically supervised use of ibogaine, a naturally occurring psychoactive compound from the root bark of the iboga plant, to interrupt opioid withdrawal, quiet cravings, and open room for deeper recovery work. At Iboga Wellness Institute, it is not a single dose and a goodbye. It is a full sequence of care: screening before you travel, preparation, a monitored treatment day, close observation afterward, and at least 12 weeks of integration once you are home.
The steps before treatment matter as much as the treatment. Careful preparation protects your safety. It tells you honestly whether you are medically eligible. And it sets you up to use an intense experience well. Iboga Wellness Institute does not treat ibogaine as a miracle cure. Its view is simple: the standard of care around the medicine is what matters most.
Ibogaine is not right for everyone. That is the first thing a responsible provider should tell you, and it is the first thing we tell you here.
Who comes to us, and what they are afraid of
Most people who reach Iboga Wellness Institute are struggling with addiction and have already tried hard to stop. They have been through detox programs, residential rehab, maintenance medications, therapy, and attempts to quit alone. Many have relapsed several times. By the time they or a family member reach out, they are exhausted, discouraged, and afraid to believe anything can help.
Two worries come up again and again. The first is whether ibogaine is really as good as people say, or just another promise that is too good to be true. The second is whether it is an underground, illegal practice.
We answer both honestly. Ibogaine does not work for everyone. Treatment at Iboga Wellness Institute is given legally in Cozumel under medical supervision. Screening and monitoring are thorough. We encourage hard questions, and we turn away candidates when the risk is too high.
The conditions we treat include opioid addiction, heroin addiction, pain pill addiction, kratom addiction, benzodiazepine addiction, withdrawal and withdrawal symptoms, trauma and PTSD, depression, anxiety, grief, and Parkinson’s disease. We also work with people who have lived with suicidal thoughts. We do not provide ibogaine during an acute psychiatric emergency or an active suicidal crisis, and we do not offer emergency psychiatric intervention.
What is ibogaine treatment?
Ibogaine treatment is a structured medical process, not a ceremony and not a shortcut. Its center is usually a flood dose: a single, larger dose given under continuous monitoring, meant to ease opioid withdrawal and reduce cravings in one session rather than across weeks of tapering. Researchers describe ibogaine as a compound with real therapeutic potential and real cardiac risk, which is exactly why the setting matters (Ibogaine: Therapeutic Potential, Cardiac Safety, and Translational Considerations).
At Iboga Wellness Institute, treatment sits inside levels of care that include detox, residential care, medication-assisted treatment, and ibogaine treatment itself. That lets the clinical team meet you where you are. Some people arrive needing a careful transition off other substances first. Others are ready to move toward treatment sooner once screening confirms it is safe.
Treatment is also built around you, not a template. We do not use fixed dosing protocols. We do not let the calendar decide when you are ready. If your labs, your ECG, or your withdrawal picture say wait, we wait.
How it compares with conventional addiction care
Traditional opioid treatment usually follows one of two paths. One is long-term maintenance on medications such as methadone or buprenorphine, which the National Institute on Drug Abuse recognizes as effective tools for opioid use disorder. The other is detox followed by residential rehab and outpatient therapy. Both help many people, and federal resources from SAMHSA remain a sound starting point for anyone exploring options.
Ibogaine treatment fills a different space. It is for people for whom those paths have not held, and who want to stop rather than maintain. It does not replace therapy, structure, or support. It tries to create a window of reduced withdrawal and clearer thinking, and then fills that window with integration work.
What does ibogaine do to your brain?
Ibogaine does not act on just one target. It and its long-lasting metabolite, noribogaine, interact with several receptor and transporter systems in the brain, including opioid, serotonin, dopamine, and NMDA pathways. That broad activity is one reason researchers believe it can soften withdrawal and cravings at the same time, rather than simply swapping one opioid for another (Ibogaine: Therapeutic Potential, Cardiac Safety, and Translational Considerations).
People often call this a “reset.” The term is loose, but the experience behind it is real for many patients. As withdrawal eases, the constant pull toward the next dose can quiet down. Many describe waking from treatment clearer than they have felt in years. Ibogaine is also known for a long, introspective phase in which memories and emotions surface. Patients often say they see their own patterns from the outside for the first time.
That same broad activity is why ibogaine demands respect. Its effects on heart rhythm, especially lengthening of the QT interval, are well documented in medical literature (UVA School of Medicine Toxicology). The mechanism that may help you is the reason every safety step below exists.
Research is also looking past addiction. A Stanford Medicine study of special operations veterans with traumatic brain injury found significant improvements in disability, PTSD, depression, and anxiety after ibogaine given with magnesium to protect the heart (Stanford Medicine). For the trauma-exposed people we serve, that line of research matters.
Your medical evaluation, before you ever travel
A thorough medical workup is the most important step before ibogaine treatment. Iboga Wellness Institute requires comprehensive screening before arrival. That includes a 12-lead ECG, labs, electrolytes, and a full medication review. A physician reviews your history before you are cleared.
Cardiac screening. Your heart has to be strong enough to tolerate ibogaine. The ECG looks for rhythm problems and QT prolongation that could become dangerous during treatment. Arrhythmias, structural heart disease, and uncontrolled high blood pressure can rule treatment out. Iboga Wellness Institute does not treat people with serious cardiovascular conditions. Treatment does not proceed if QTc is over roughly 460 ms. In our own experience, roughly 5 percent of applicants are declined based on EKG results alone.
Blood work and electrolytes. Labs check liver and kidney function, since the liver metabolizes ibogaine. Electrolytes get special attention because potassium and magnesium directly affect heart rhythm. Iboga Wellness Institute targets potassium of 4.2 to 4.8 and magnesium of at least 2.0 before treatment.
Urine toxicology. Urine screening detects benzodiazepines, buprenorphine, alcohol, and other substances that could complicate treatment. It also screens for pregnancy. These results guide your detox plan and your dosing.
Psychiatric fit. We do not treat people with psychosis or certain bipolar presentations. We do not offer ibogaine to someone in an active suicidal crisis. Applying or paying does not guarantee treatment. Those boundaries protect you.
Medications, supplements, and what has to stop first
Everything you take matters, prescribed, over the counter, or recreational. Some substances are not compatible with ibogaine and raise risk sharply.
Drugs that increase serotonin, such as SSRIs, SNRIs, MAOIs, and certain pain and migraine medications, can raise the risk of serotonin syndrome. They usually need to be stopped well before treatment, and any taper belongs with your prescribing doctor. Some substances also block CYP2D6, the liver enzyme that processes ibogaine. These include some antidepressants such as paroxetine, grapefruit products, and some antiarrhythmics, antipsychotics, antibiotics, and beta blockers. When that enzyme is blocked, ibogaine can build up to unsafe levels.
You will likely be asked to avoid caffeine from coffee, tea, energy drinks, and chocolate for several days before treatment, since it can lower potassium and unsettle heart rate. Herbal products that affect rhythm, blood pressure, or the liver need clinical clearance. An honest, complete list is one of the most protective things you can bring us.
Opioids and maintenance medications. If you take methadone or buprenorphine, you will usually need a supervised transition to short-acting opioids before ibogaine. That is never a do-it-yourself taper. Iboga Wellness Institute builds personalized taper plans for this stage.
Alcohol and sedatives. Heavy or daily drinking usually requires medically supervised detox before ibogaine, especially if you are at risk for withdrawal seizures. Benzodiazepines and other sedatives often need a careful, supervised taper as well.
How is ibogaine administered?
At Iboga Wellness Institute, the treatment sequence runs in this order:
- Pre-arrival screening. You complete a 12-lead ECG, labs, electrolytes, and a medication review. A physician decides whether you can move forward.
- Preparation and tapering. You join pre-treatment preparation sessions and follow your personalized taper plan for opioids, alcohol, or other substances.
- Arrival screening. On arrival, the team repeats the ECG and labs and runs two urine drug screens to confirm nothing has changed.
- Electrolyte correction. Potassium and magnesium are brought into target range. IV magnesium is given as a cardioprotective protocol.
- Consent and settling in. You review and sign informed consent, confirm emergency contacts, and surrender unapproved medications and substances.
- The flood dose. Ibogaine is given with continuous cardiac telemetry, 24/7 nursing, physician oversight, and emergency equipment ready. The most intense phase often lasts many hours.
- Close monitoring. Monitoring continues for 24 to 72 hours after treatment while your body and heart rhythm settle.
- Integration. Weekly one-on-one or group integration continues for at least 12 weeks after you leave.
One patient’s journal from recovery reads, “I cannot believe how at peace I feel.” He wrote that his cravings had stopped. His vitals were checked three times a day: heart rate around 70, blood pressure around 125/85, and QTc around 435. That rhythm of checking is ordinary here, and it should be ordinary anywhere ibogaine is given.
What makes Iboga Wellness Institute different
Iboga Wellness Institute is a physician-led program built to raise the standard of ibogaine care. The difference shows up in the specifics. Screening happens twice, before travel and on arrival. Electrolytes have defined targets, not guesses. Cardiac telemetry runs continuously through the flood dose. Nurses are present around the clock. A physician oversees treatment. Monitoring lasts days, not hours.
The institute describes a 4:1 staff-to-patient ratio and two private Cozumel settings, Ocean Sanctuary and IWI Reserve, held to one clinical standard. It also describes itself as a US-based institute, which matters to American families who want familiar accountability. Comfort matters, because healing is hard work. But comfort never outranks the monitor.
We have been open about 10 months. We say that plainly because honesty is the point. A young program that screens hard, declines unsafe candidates, and tracks its follow-up is worth more than a long history of cutting corners.
What is the success rate of ibogaine treatment?
There is no single, established success rate for ibogaine treatment. Results vary by person, and large controlled trials have not yet set a reliable figure. Anyone quoting a guaranteed number is overselling.
What exists is promising research and firsthand outcomes. The Stanford study of veterans with traumatic brain injury reported significant drops in PTSD, depression, and anxiety symptoms, along with better daily functioning, one month after treatment (Stanford Medicine). Reviews of the broader literature describe meaningful effects on opioid withdrawal while stressing that cardiac safety must shape any treatment model (Ibogaine: Therapeutic Potential, Cardiac Safety, and Translational Considerations).
At Iboga Wellness Institute, success is measured over months, not on treatment day. Zach is more than seven months out. He is sober, back working full time as a cook, has repaired family relationships, and still attends weekly integration. Nigel Hope arrived contemplating suicide. Months later he is sober and has reconnected with his family. The institute itself began with an Army veteran friend who was living with suicidal thoughts. His flood dose lasted about 8 to 10 hours, and he woke clearer, with reduced cravings and withdrawal.
These are real stories, not promises. They show what becomes possible when a careful treatment is followed by sustained support.
Is ibogaine legal in the United States?
No. Ibogaine is a Schedule I controlled substance under federal law in the United States, and it is not FDA-approved for any medical use. Iboga Wellness Institute never claims otherwise.
That is why many Americans travel for treatment. Iboga Wellness Institute provides treatment legally in Cozumel, under medical supervision. Some US states have started exploring ibogaine research, and federal agencies such as the National Institutes of Health track ongoing study of psychedelic compounds. Interest is growing, but research is not the same as legal access.
Legal access abroad does not mean every provider is safe. When you compare options, look for on-site medical staff around the clock, real pre-treatment testing rather than a short questionnaire, repeat testing on arrival, continuous cardiac monitoring, a willingness to say no, and a structured plan for aftercare. Ask who will be watching your heart, and for how long.
How much do ibogaine treatments cost?
The cost of ibogaine treatment at Iboga Wellness Institute depends on your medical needs, your length of stay, and your program setting, so your quote follows screening rather than coming from a flat rate. Ibogaine treatment is not FDA-approved in the United States, so families should plan for treatment to be largely self-funded and confirm any coverage questions directly.
Several things shape the final number. Some people need a longer stabilization or taper before treatment. Some need extended monitoring afterward. The choice among the two Cozumel settings changes the experience, while the clinical standard stays the same. And the price at Iboga Wellness Institute includes the parts many people do not see in a low quote: two rounds of screening, repeat ECGs and labs, urine toxicology, IV magnesium, continuous telemetry, 24/7 nursing, physician oversight, days of post-treatment monitoring, and at least 12 weeks of integration.
When you compare costs anywhere, ask what is actually included. A low price that leaves out cardiac monitoring or aftercare is not the same treatment.
Preparing your body and your mind
Your physical condition affects how your body handles ibogaine. In the days before treatment, steady hydration supports your liver and circulation, unless your doctor directs otherwise. Balanced meals, consistent sleep, and avoiding alcohol and heavy processed food all help. Keep potassium and magnesium in mind, since they protect heart rhythm.
Mental preparation matters just as much. Ibogaine is a long, introspective experience. Many people benefit from journaling about three things before they come: their past with substances and trauma, their life as it is now, and the life they want next. A few quiet minutes each day practicing simply noticing thoughts, without reacting, can make long introspective hours easier to move through. You do not need any particular spiritual belief. Iboga Wellness Institute’s preparation sessions help you do this work with guidance.
Your environment needs attention too. Before you travel, delete dealers’ numbers, leave group chats built around using, and clear your home of paraphernalia and hidden supplies. Choose two or three trusted people to be your safe circle after treatment. Be honest with them about your history and what you will need when you return.
Aftercare that does not end at the airport
The weeks after ibogaine are often called a window. Cravings may be lower and thinking clearer, but old habits and old places still wait at home. Iboga Wellness Institute provides post-treatment follow-up and weekly one-on-one or group integration for at least 12 weeks. Integration helps you turn insight into daily practice: new routines, repaired relationships, and honest check-ins when things get hard.
Plan a quiet, low-stress return with time off to rest. Avoid alcohol for at least the first week after treatment, and longer as your team advises. Lean on your safe circle. Keep your integration sessions, even on the days you feel fine. Zach still attends his.
For families, this is often the part that matters most. You are not sending someone away for a single event. You are choosing a program that stays with them afterward.
